Where to report

All work injuries must be reported to the supervisor.
The supervisor or other responsible party must notify EH&S immediately at (510) 642-3073 in the event of a fatality, or serious injury or illness that results in in-patient hospitalization, loss of any body part(s), or permanent disfigurement. If the injury or illness occurred after business hours, call UCPD at (510) 642-3333.
The supervisor shall report the injury or illness through the Injury and Illness Reporting (IIR) website(link is external) within 24 hours of the incident.

Do I also need to make a report elsewhere?

To report workplace-related sexual violence or the discrimination/harassment of a member of a protected category, submit an online report or contact the Office for the Prevention of Harassment and Discrimination (OPHD) at 510-643-7985 or ask_ophd@berkeley.edu.
