Abusive conduct (including violence) in the workplace reporting

Informal summary: The UC Policy on Abusive Conduct in the Workplace prohibits abusive conduct (e.g. bullying) by employees in the workplace. Abusive conduct is harassing or threatening behavior that is severe, persistent, or pervasive.

Workplace violence reporting

Informal summary: Violence, and threats of violence, are prohibited in the University of California, Berkeley workplace. Anyone who observes such behavior or believes a credible threat of such behavior exists, or is a victim of such conduct, is encouraged to immediately report it to the University. Managers and supervisors who observe or learn of such conduct are required to report it to the University.

Sexual violence and sexual harassment (SVSH) reporting

Informal summary: The UC SVSH Policy prohibits sexual violence and sexual harassment, including but not limited to sexual assault, sexual harassment, relationship violence (dating and domestic violence), stalking, sexual exploitation, invasions of sexual privacy. Non-confidential employees of the University must notify OPHD if they learn, in the course of employment, about an incident of Prohibited Conduct.

Protected Category discrimination and harassment reporting

Informal summary: The UC Anti-Discrimination Policy prohibits harassment and discrimination based on an individual’s actual or perceived Protected Category, as well as retaliation. Protected Categories include but are not limited to race, religion, citizenship, national or ethnic origin, ancestry, sex (including pregnancy and related medical conditions), gender, sexual orientation, physical or mental disability, age, and veteran or military status.

Improper governmental activity reporting

Informal summary: If you suspect that a UC employee is engaged in improper governmental activities, you can report ("blow the whistle"). Improper governmental activity includes, but is not limited to, corruption, bribery, fraud, or the waste or misuse of University resources. Other kinds of misconduct can also be reported to the UC Whistleblower Hotline.

Serious injury/illness in the workplace reporting

Informal summary: All work injuries must be reported to the supervisor. The supervisor or other responsible party must notify EH&S immediately at (510) 642-3073 in the event of a fatality, or serious injury or illness that results in in-patient hospitalization, loss of any body part(s), or permanent disfigurement.

Crime reporting (including Clery crimes)

Under the federal Clery Act, people with campus security responsibilities must report those crimes covered by the Clery Act (including but not limited to motor vehicle theft, robbery, burglary, hazing, sexual assault, domestic and dating violence, and stalking) for inclusion in the daily crime log and annual statistical reporting. lease see the Clery Act Compliance website for a comprehensive list of Clery crimes and definitions.

Hazing reporting

Where to report In an emergency or if there is a serious ongoing threat Violence and other criminal hazing activities should be reported to UCPD.
